Consumer health data privacy policy
Version 26 October 2026
This is Part B of the Elevate Golf Privacy Policy, published separately because Washington and Nevada consumer-health privacy law requires a distinct notice. The full Privacy Policy explains all other processing.
This part concerns consumer health information processed by Elevate Golf International Limited where Washington or Nevada consumer-health privacy law applies. It explains the health information involved, its use and sharing, and how to exercise the applicable rights.
H1 Health information and sources
We collect optional injury, medical-condition, physical-limitation, allergy, dietary and related physical or mental-health information supplied by you or your authorised Guardian through profiles, requests, recordings or messages. Relevant Coach exchanges and limited inferences from that information may also reveal health information. Account identifiers link the information to the person. An ordinary golf recording is assessed by its contents and use.
H2 Purposes and sharing
We use this information to support suitable requested golf advice and necessary safety. A direct request shares relevant information with the selected eligible Coach. A permitted posted request can share it with compatible eligible Coaches considering an Offer before selection or purchase. Required collection and sharing permissions are obtained separately where the law requires.
Contracted infrastructure and storage, communications, diagnostics, support, AI and transcription providers receive health information only where needed for the disclosed service. These are the third-party recipient categories; the relevant route determines which receive it. We do not share consumer health information with corporate affiliates for their own purposes. Authorities and professional advisers receive information only under a lawful requirement or justified exception.
We do not sell health information or use it for advertising, commercial audience reports or model training. We do not collect precise location to infer health-facility visits, or authorise third parties to collect health information across other websites or services for their own tracking purposes.
H3 Choices and requests
Email privacy@elevategolf.com or use the app’s privacy route to confirm covered processing, request access and applicable recipient information, withdraw consent or request deletion. A requested-service exception applies only where its actual conditions are met. We verify identity proportionately and do not discriminate unlawfully against people exercising rights. Deletion covers applicable processors, recipients, archives and backups, subject to the relevant law and lawful exceptions. Our 90-day maximum backup period does not extend a shorter applicable duty.
H4 Washington rights
We respond to covered requests within 45 days of receipt. If a permitted extension is necessary, we explain the reason within the original period; the extension is no more than a further 45 days. You may appeal a refusal through privacy@elevategolf.com. We decide the appeal within 45 days of receipt and give reasons. If refused, we provide information on complaining to the Washington Attorney General at atg.wa.gov.
H5 Nevada rights
You can request confirmation of covered processing, relevant third-party recipient information, cessation of covered collection, sharing or sale, and deletion. General requests are answered without undue delay and within 45 days of authentication, with a further 45 days only where permitted and explained. Covered deletion and required notification to recipients are completed within 30 days of authentication; notified recipients have their own applicable 30-day deletion duty. Appeals sent to privacy@elevategolf.com are decided within 45 days of receipt. We explain any refusal and provide the Nevada Attorney General complaint route at ag.nv.gov.
